Adding an Immunization Record

To add an immunization record to a child follow these steps.

1. Select Menu and then Manage Family

2. Select Children

3. Select child you would like to upload the record for

4. Scroll to bottom of page

5. Click on the Upload Immunization Document button

6. Attach the file or add from your camera (make sure any photos of document are clear and easy to read)

7. Save the page

8. Wait for the upload to complete where it will take you back to child selection.
